To Claim Rebate:

1. Purchase and register a qualifying Green Dot Reloadable Prepaid Visa or MasterCard (a "Card") from a participating Rite Aid or CVS (each, an "Authorized Retailer") between July 1, 2011 and September 30, 2011, to receive a $4.95 credit onto your Card. This rebate offer does NOT apply to Green Dot Reloadable NASCAR Visa Cards. The rebate request form ("Rebate Request Form") must be obtained directly from a Green Dot display found at an Authorized Retailer, while supplies last.



2. To receive a $4.95 credit onto your Card, between July 1, 2011 and October 31, 2011 mail in an envelope with proper postage affixed, a fully completed rebate claim request (collectively, with (a), (b) and (c), the "Rebate Claim") containing all of the following: (a) the ORIGINAL cash register receipt from your Card purchase dated between July 1, 2011 and September 30, 2011 and imprinted with the store name and Card purchase/load amount circled (b) the ORIGINAL barcode found on the back of the Card package (c) the ORIGINAL completed Rebate Request Form (which must be obtained at a participating Authorized Retailer, while supplies last; for a list of participating Authorized Retailers, visit www.greendot.com) with valid name, street address, city, state and zip, telephone number (including area code), e-mail address, and the Unique Package number found at the top of prepaid Visa or MasterCard package, to:

   The check box confirming that you are 18 years or older must be checked. Card must be registered and active at time of rebate or $4.95 credit will not be received and rebate will be forfeited. Rebate Claims must be postmarked by October 31, 2011 and received by November 7, 2011 to be eligible for the rebate. Please make a copy of your receipt for your own personal records. Rebate Claims must be made via U.S. mail.

   Terms & Conditions: Cards and credits are subject to certain terms and conditions specified by issuer, available at www.greendot.com. Cards may be used to purchase goods and services anywhere Visa or MasterCard debit cards, as applicable, are accepted and to access cash at ATMs of financial institutions displaying the Visa or MasterCard name and/or logo (ATM fees may apply.). Cards expire on the dates specified thereon. Certain fees will apply with respect to the use of Cards. Offer valid only in the USA for purchasers who are legal residents of the 50 United States or the District of Columbia, 18 years of age or older. All Rebate Claims must be postmarked by October 31, 2011 and received by November 7, 2011. Limit ONE (1) Rebate Claim and $4.95 credit per Card purchase (including this rebate and any other rebates currently running through Green Dot Corporation ("Green Dot"). This rebate offer cannot be combined with any other offer. Rebate Claims must contain all components listed in section #2 above to be eligible. Except for residents of North Dakota, Rebate Claims from PO Boxes will not be accepted Rebate Claims with invalid, undeliverable and non-U.S. mailing addresses will be void. Green Dot and its parent, affiliates, subsidiaries, advertising and promotion agencies, retailers, suppliers, distributors and each of their respective employees, officers, directors, agents and shareholders (collectively, the "Released Parties") are not responsible for printing errors or for lost, late, stolen, incomplete, damaged, delayed, illegible, garbled, mutilated, misdirected or postage-due Rebate Claims, credits or mail and all such Rebate Claims, credits or mail will be void. Incomplete or illegible Rebate Claims, mechanically reproduced Rebate Claims, and Rebate Claims generated by any means which subvert the rebate process will not be honored. This rebate offer is void where taxed, restricted or prohibited by law. No substitutes, transfers or rain-checks are permitted. Rebate Claims become property of Green Dot and will not be returned. Released Parties are not responsible for errors of any kind whether human, typographical, printing or otherwise relating to or in connection with the rebate, including, without limitation, errors which may occur in connection with the administration of the rebate, the processing of Rebate Claims or credits, or in any rebate-related materials. Persons who tamper with or abuse any aspect of the rebate, as solely determined by Green Dot, will be disqualified. Should any portion of the rebate offer be, in Green Dot's sole opinion, compromised by non-authorized human intervention or other causes, which, in the sole opinion of Green Dot, corrupt or impair the administration, security, fairness or submission of information, Green Dot reserves the right at its sole discretion to terminate the rebate. By participating, you agree (i) to be bound by these terms and conditions and the decisions of Green Dot which are final and binding, and (ii) to release and hold harmless Released Parties against, any liability, for any injuries, losses or damages of any kind, including death, to persons, or property resulting in whole or in part, directly or indirectly, from participation in this rebate. No claims against "lost" materials will be honored unless accompanied by proof of receipt (e.g., certified mail). Fraudulent submissions and/or use of multiple addresses or PO Boxes to obtain multiple rebates could result in federal prosecution under U.S. mail fraud statutes (Title 18, USC Sections 1341 & 1342). Green Dot reserves the right to substantiate Rebate Claims and request additional identification and/or documentation. Rebate credit will be credited to claimant's card within 6-8 weeks of the receipt of eligible Rebate Claim. Rebates are processed by a third-party rebate fulfillment company. Only actual purchasers of the product may participate in this rebate offer. Rebate offer is not valid for groups, clubs or organizations, or for employees of Green Dot, the rebate fulfillment company, or their respective parents, affiliates, subsidiaries, advertising and promotion agencies, retailers, suppliers and/or distributors. If any part of these Terms and Conditions is deemed to be invalid or otherwise unenforceable or illegal, the balance of the Terms and Conditions shall remain in effect and shall be construed in accordance with its terms as if the invalid or illegal provision were not contained herein.
